Details of the accident/Incident: Scaffold Structure Collapse.
Investigation taken from eye witness: According to witness Excavation work was ongoing near the scaffold structure, and due to a cave-in beneath the base plate, the scaffold collapsed.
Decision: It was totally mishandled by the excavation team and the engineer. They gave permission for excavation near the structure.
Close: I am closing the investigation based on this decision.
